An investigation into a tragic accident in Grindavík reveals serious concerns about safety protocols and project management. On January 10, 2024, a worker involved in filling fissures fell into a gap and was later declared dead. The report by the Occupational Safety and Health Administration questions whether the project posed an acceptable risk given the ongoing geological instability in the region.
According to the findings, there was a lack of adequate risk assessment, particularly concerning geological conditions, when the work began late last year. The victim, Lúðvík Pétursson, was working alongside a colleague when the accident occurred. The project had experienced disorganized management, with workers switching tasks based on conflicting demands from different stakeholders, including local town representatives. This disarray extended to the oversight of tasks, with workers sometimes juggling multiple roles under various supervisors.
Initial earthquake tremors in November 2023 had prompted the work to secure fissures in the area, with a geological firm commissioned for the project. However, operations were halted when volcanic activity began in December. When work resumed in January, Lúðvík was using a portable earth compactor, as the primary machine had malfunctioned. Unfortunately, he fell into a fissure during his work.
The report highlighted that proper fall protection was not in place, and while the engineering firm was aware of the equipment failure, they did not implement a plan to use safe machinery. In conclusion, the Occupational Safety and Health Administration stressed that thorough risk assessments should be standard practice and called into question the necessity of the project given the risks involved. Lúðvík, who was 50 years old and a father of four, lost his life in this incident, raising critical discussions surrounding workplace safety in hazardous environments.
