Päijät-Häme wellness region secures €1.2 million for continuity of care

The Päijät-Häme wellness region has received a €1.2 million state grant to improve continuity of care, Yle reports.

Under the initiative, patients will be assigned a dedicated doctor and nurse to oversee their treatment needs and long-term condition monitoring. The model has already been implemented in approximately 15,000 cases across municipalities including Hollola, Asikkala, and Heinola.

The funding, allocated for 2026–2027, will support the development of digital tools, monitoring methods, and staff training. According to Pertti Sopanen, head of outpatient care services, familiar healthcare providers streamline care access and reduce redundant work.

Some consultations may shift to remote formats, freeing in-person appointments for those requiring direct contact. Sopanen noted that non-urgent communication via digital channels will increase, reducing the need for phone calls.
